Dark Powers
The true force behind the Mists and prison guards of the Demiplane are a mind-bending collection of beings. The first time they made an appearance was in offering Strahd von Zarovich a way to everything he ever wanted... only to rip out the rug beneath his feet. Since then, they've appeared whenever a creature began to approach the threshold of too evil to live and far too evil to be allowed a peaceful death and snatched them away.
To achieve their goals, the Mists can be used to bind the domains to form the demiplane and as fuel for the curses and abilities of the darklords, fuged, and mistborn. General "upkeep" of the fabric of reality is also within the Mists' purview, such as pretending to be the different gods and goddesses worshiped and the recycling of souls.
When the Mists don't do and a more human seeming touch is needed, the Manusa Tasque and Madame Eva appear to give events a nudge. Madame Eva herself is quite a legendary figure in the history books, always appearing as the same ancient crone offering glimpses of the future to those needing guidance towards the correct decision. Exactly who is in charge between the Powers and Madame Eva is up for debate, but the results don't care.
Structure
Two entities hold potential for being declared "leaders": Madame Eva and the consciousness running the Mists. Each directs and controls the Vistani and the Fuged/Mistborn respectively.
Public Agenda
The Powers are focused on the goal of trapping and containing those deemed dangerous to their home world/plane. As a result of Madame Eva's influence, they have also managed to bend their abilities so as to best extend the suffering of the prisoners, taking in whatever is deemed "necessary" with the Mists, such as heroes to battle them and highlight their failings.
History
In their early days, they simply gathered power, siphoning off from deities focused on justice and trickery. The deal with Barovia's darklord gave them the needed stepping stone to create a prison for all those they wished to capture and, when Tatyana fell to her death, her only true friend was able to reach out and twist the magic of the deal, allowing for her to insert herself as an equal of the Powers in the deal.
