All mortal realms are floating in the Astral Sea. Clusters govern which worlds are closest to one-another.
Nexus points within a world will always be able to connect to other worlds in the cluster, but rare are those that will connect to worlds outside the cluster. So to get to a distant world by the use of Realm-gates you may have to trek through several worlds to get to your destination.
