Usually self appointed, but in the case of The War of 1471 the king (King Jon Reed II) was elected by his Lord Paramounts whilst in the halls of Isenhold.

Day to day running of a kingdom and maintaining diplomatic relations with other kingdoms/empires.

They can be dissolved or over thrown (such as The coup of 13th of December 1472, The chaotic coup)
History
Many of the ancestors of the current Lord Paramounts of The 14 Kingdoms where themselves kings. However after the unification of 1004 (creation of The 14 Kingdoms) they were demoted to Lord Paramounts.
Cultural Significance
Apart from Emperor, this is the highest possible rank. It holds massive cultural weight.
