Malpheus is one of the Demonlords of Rancor, former ruler of the First Layer of Hell and the Lord of Calamities. Often taking the appearance of a Nilfanel elf, nothing is known of Malpheus before his fall into the pit, and then ascension as a Demonlord of Hell during The Godswar. He served Belegor faithfully during his tenure as the King of Hell, but quickly switched sides after Belegor's fall, swearing allegiance to Belros Azul as his sovereign.

During the Infernal Crusade Malpheus led a demonic horde on Earedar, landing at the settlement now known as Maldeus. Malpheus led the campaign against the human kingdoms of the Westhold, as well as the High Kingdom of the Stonelands and the Second Vraekar Empire. Malpheus' inability to conquer these lands greatly frustrated Belros, as his inability to reinforce Yzgaroth and Baphomet, combined with Aeltruad's failing in the Elswyld prevented the forces of The Shadow from a quick victory.

Malpheus' final failing in the eyes of Belros came during the event known as The Calamity in 6697-6698 CY. Wishing to regain the favor of the Prince of Darkness, Malpheus found a way to tear the veil at the Shattered Throne on midsummer's eve 6697 CY, and led a demonic horde through the portal onto Earedar, the first horde to walk upon this soil in almost seven thousand years. Incensed that he did not share his plans ahead of time, Belros was even more enraged at the defeat of Malpheus in 6698 CY at the Battle of Shadowglen at the hands of Balthazar Blake and Zenathar Blake. Belros would subsequently remove Malpheus as the ruler of the First Layer of Hell, and replace him with the mysterious figure known as Melkor, the Lord of Pain. While Melkor has since been revealed as none other than Zenathar himself, fallen after the Battle of Shadowglen and subsequently saved during the events of the Fall of Illidion, Malpheus has not been restored to his place as the ruler of the First Layer of Hell, instead replaced on an interim based by Djzargo, Steward of Hell.

Circle of Shadows- Most prominent within the Felidar Imperium, Yvren and other places where the Shadow holds sway, the Circle of Shadows is comprised of a cabal of warlocks, clerics, priests and other mages who draw upon the Shadow for their power. Fueled by raw ambition, members of the Circle are driven to accumulate as much power as possible, often at the expense of others, including their own members. They are often cruel and unforgiving, and can be found acting as priests amongst thsoe regions controlled by the forces of darkness. They venerate Belros Azul above all others as the pinnacle of their ideals.
Cult of Calamity- Fervent followers of Malpheus, the cultists of this group worship him as the embodiment of destruction. These cultists believe Malpheus is responsible for all the natural disasters of the world, and only be appeasing him with gifts and sacrifices will he bless them with infernal power.
