Subtle, a true blessing for Their prophet and me. This variant isn’t as drastic as Puppeteer but accomplishes a similar goal. Those affected seem to turn to our Mistress’ goals. It is no surprise that the amount of acolytes steadily increased.|Mazil
While Mazil was stating the variant isn't drastic, it seems to be entirely the opposite. Instead of any outwards signs the fungus creeps and spreads inside the host body centering around the head. Its target seems to be the brain. it wouldn't surprise me if we found the brain replaced by fungus completely.
Once the fungus has completely taken over the patient becomes a husk that vacantly stares into the distance and does not move. (probably until the fungus hive mind decides to do something but not sure about this)

Diagnosis

The fungus can be seen as fungus balls in the afflicted's mouth and nostrils.
