Incident reconstruction:
Manipulation by hand did not occur in a plausible time frame to be of concern.
Affected components were not accessed since construction.
Individual components do not indicate a cause for machine failure.
Madlinson Refiners Industry heavy-duty lubricant was used throughout the factory.
There is no information on the condition of the lubricant at the time of the occurrence, though other machinery feeding off the same supply continued working uninterrupted.
The mechanism heated up in a very short period of time after an insect was observed flying through the machinery. The insect could not be examined since it was exterminated unwillingly by a member of staff. The member was not detained since the estimated time of imprisonment for the offence of obstruction as defined by EE1-D would have been under the minimum imprisonment limit of 6 hours as required by
the Book of the Crown's Justice. Should the importance of the offence be re-evaluated to prolong the sentence, the individual will be treated as a
Class 0 Offender until relieved or the termination of the investigation, whichever occurs more promptly, as specified in the Book of the Crown's Justice. The offender has been informed of their deed and that there should be no consequences in the foreseeable future.
