The Scar

The Scar is a rift within the Eldorani Crater, formed at the end of the Second Age through the Black King's ritual of lichdom. The energies unleashed through the ritual shattered the planet at his location, driving a spike of arcane power straight down. The resulting rift created an opening into the Underdark, the only publicly known entrance into this sub-plane.
Due to his location, Leander's mask, undamaged by the ritual, fell into the Underdark through The Scar. There it remained for many thousands of years until it was discovered.
