Deck of Foretelling
A deck of beautifully painted and hand crafted fortune cards, twenty of them in totaly, alive with magick. A deck of promise and pain, of fortune and failing. A deck of risk and reward. Those whom wield this gift of the Gambler can find much boon, if they are lucky, if Tomain smiles favorably upon them. But like any artifact so blessed by the Eternal Gambler, there is no reward without risk.
It is believed that there are less than twenty of these decks in existence all across Valerick, they were once great gifts, given to winners of tournaments and challenges set by the Eternal Gambler himself and now they pass through the world, discovered on occasion and wielded by mortals to their benefit, or their great harm. Such is the double edged nature of Tomain. He does not give freely, all life is meant to be a game of chance, and such is true of such gifts. Those whom attune to such a deck find such things soon enough.
This is a magick item, it requires an attunement slot. 1/long rest (or 1/day for those whom run games that way) the attuned individual may pull a card from the top (roll a d20). Once pulled a card is consumed, removed from the deck forever. Should an attuned individual give up their attunement from a deck that has had even one card pulled from it, the deck entirely will disappear, the game over. The cards and their effects are listed below;

4 | The Debt Collector | You must give up 1 object of value equal to 100 gold*your level to debt collection, it simply fades away with the card. |

6 | Lady Luck's Kiss | You've been touched by Lady Luck herself. You bank a natural 20 and at any point in the day you may simply substitute rolling a die for that natural 20. This boon fades away at the end of the day (so when you long rest again for 5e types) |

8 | The House | If you had no luck, it would be better. You are at disadvantage (or -4 to all you PF2e types) to any saving throw for the day |
9 | Gambler's Delight | You've always gotten lucky when its all on the line. You have advantage (+4 to all you PF2e types) to any saving throw for the day. |
10 | Favored Caller | You are incapable of failing Prayer (Varis Tomain) skill rolls for the day. If you normally would not be able to call upon such boons, you gain access to two Prayers of Casting TN=20 or less for the day. (For 5e/PF2e, simply give out 1 extra 1st and 2nd level spell slot to divine casters or allow someone to cast a 1st and 2nd level divine spell of their choice, or that you choose that are thematically appropriate, for the day) |

12 | Blood Sport | You play some of the greatest game, gambling with that most precious, your life. For ever wounded condition you are carrying, you gain +2 to every d20 roll. (For 5e, I would portray this as +2 for every 1/4 their hit points they are missing) |
13 | Misfortune's Kiss | You've been kissed by a spirit of ill fortune. Your first natural 20 of the day becomes a Natural 1. |

15 | Silver Tongued Savant | You understand part of the great game is not just playing it, but playing the players. You gain +2 to all Charisma based skill checks for the day. |

17 | Jackpot! | You gain a special reaction you may use up to 4 times this day. You can declare 'Jackpot!' and turn any ally within 60 feet of you's d20 roll into a Natural 20. |
18 | A Debt of Blood | For the day your Maximum hp drops by 3/character level, or by 1/3 whichever is less. |
19 | The Finest of Prizes | You find yourself in possession of a Magick item worth at least 4000 gold. This object will fade after the day ends (when you next long rest) but it is yours to utilize as much as you wish for the day. |
20 | The Greatest Prize | You may make a single limited Wish. The limitations are based upon Tomain's influence and will be laid out by your GM when you draw this card. For GMs the limitations should exist but shouldn't be crippling this card is the ultimate prize, its why people risk this deck at all. |
