Be the Book

Your body is that of a book, printed long ago in a place of great magic. Though you have the power to slip between forms. If your book was ever to be destroyed, there would be no easy way to bring you back.
You have three forms, that of a Book a Humanoid and a Monster Book. Your Book form is your primary form and your Monster Book form is a mutation of this. Your humanoid form uses your standard hitpoints, but your book forms have their own pool of hit points equal to your characters total level + your Constitution modifier. If your humanoid for is destroyed it may reform after you have completed your next long rest. But if your book form reaches zero hitpoints your character is slain immediately without the chance of any saving throws. Though they may still be revivified or resurrected as normal in line with the setting you are plying in.   Only your humanoid form can change shape and your Book forms cannot be the subject of any shape-changing spells or abilities such as polymorph or wild shape. Your Book forms can still switch between each our as detailed later on.

Popup Book

Given sufficient space, you may burst forth from your booky vessel.
As a bonus action, you may leap from your pages in the form of a humanoid! You can take on the form of any small or medium humanoid but you do not benefit from any of their features or stat. This humanoid form and any of its subsequent transformations, have the appearance of a painted or drawn figure almost as if an illustration had jumped out of the pages of a book.   Your humanoid form uses your standard hitpoint pool instead of your books forms.   So long as you are able to open your mouth/pages. Using a standard action your humanoid form may leap back into your book. Any items, armour or equipment staying with your humanoid for later use.

Made of Paper

Due to the nature of your material, you are vulnerable to both Fire and Slashing Damage. But you are resistant to Bludgeoning Damage.

Surprise!

There are advantages to being so unassuming, not least of which is that your prey will often come to you. While in book from you look to all the world as a normal book.
If any creature attempts to discern your true nature you may make a deception check with an advantage against their roll to investigate. If a creature attempts to read or open you in this form without them being aware of your true nature, you may make a bite attack against them as a standard action. If this attack hits you add twice your level to the damage roll along with your other modifiers, with this damage being based on your bites primary damage type.

Monster Book

Your humanoid form is not your only trick. You may take on a more monster-like form than even a book. perhaps a form more testament to what you truly are.
As a standard action, you may take on the form of a monstrous book. While in this form you use your books hitpoint pool, resulting in instant death if reduced to zero hit points. However, you also gain a number of temporary hit points upon transformation equal to your character level. It is a standard action to change back into your more benign form, losing any temporary hitpoints you have gained in the process. Upon reaching 5th level transforming between your stander and monster book form becomes a bonus action. Temporary hitpoint gain by this ability does not heal any damage that has been done to your book form.   While you are in your Monster Book form you gain access to 30ft. of spider climb and a powerful bite attack:
To Hit: 1d20 // On HIt: 1d8+Str. This becomes a bonus action upon reaching level 6.
