A horned one is a 14th level cleric spellcaster using Wisdom (saving throw DC 13 & hits with spells on AC 13). It has the following spells.

At will: Cantrips: Blade Ward, Minor Illusion, Vicious Mockery

1/day: 4th Level: Compulsion, Dimension Door, Confusion. 5th Level: Dominate Person, Geas, Mislead.

2/day: 2rd Level: Blindness / Deafness, Crown of Madness, Phantasmal Force. 3rd Level: Bestow Curse, Dispel Magic, Stinking Cloud.

3/day: 1st Level: Bane, Disguise Self, Dissonant Whispers, Thunderwave.


Keen Smell. The horned one gains advantage on Wisdom (Perception) checks that rely on smell.   Poisoned attacks. Each of the attacks carried out by the horned one is poisoned and the same rule applies to all attacks actions. For every hit, the creature has to make a DC 16 Constitution saving throw. On a fail, the creature takes 14  2d12+1 poison damage. On a success, hit takes half the poison damage.   Inspiring Presence. The horned one buffs any ratmen around it. Every ratman within 15 feet of it adds a +1 to all attack rolls, and saving throws.

Actions

Multiattack. The horned one makes two attacks per turn: one bite, and one claw or one greatsword attack.   Bite. Melee Weapon Attack +6 to hit, reach 5 ft, one target. Hit 12 2d10+1 piercing damage.   Claw. Melee Weapon Attack +6 to hit, reach 5 ft, one target. Hit 10 1d12+2 slashing damage.   Greatsword. Melee Weapon Attack +6 to hit, reach 5 ft, one target. Hit 8 2d6+2 slashing damage.

Horned Ones are also known as Horned Rats. They are the daemons of the Horned One worshipped by the ratmen / skaven. Each horned one is a tall, lithe, humanoid creature with the head of a rat, and two sets of horns (one usually long & slender, and the other curled around the back of the ears like rams horns). Strong and imbued with diseases, poisons, and magic, they make for powerful opponents. Many carry a great sword dripping with diseased venom, and to the ratmen, they drive these mortals to acts of atrocities against all other life.
