Planar Rift / Portal

A planar rift is a direct gateway to an elemental plane. It usually requires an appropriate nexus, but that requirement can be bypassed on Midwinter Night.
A portal is similar but connects two places on the same plane. Portals are more subtle and difficult to create than planar rifts.
