Following a riot in one of formerly inrithian city, now controlled by Khem, a war between Khem and Inrithia occurs.
Being forced to give up part of their territory - small, perhaps, but still - was a significant blow to inrithian pride, especially due to being forced to do this without fighting.
Fourth Eastern War was thus a war many years in making, that was sure to occur sooner or later. The final trigger was a riot in one of annexed cities, which was put down brutally. This lead to outrage throughout the Inrithia, forcing the king (reluctant to the whole idea due to the fear of hetarian intervention) to declare the war to retake lost lands.
War ends with inrithian victory. Hetarian didn't intervened, busy with recuperating from Third Eastern War and First Hellanan War.
