Lycanthropy

This curse will turn the affected victim into a were creature if the affliction is not removed by the first full moon after they are afflicted. 
On that full moon, they will involuntarily change into a hybrid form of themselves and the type of creature they were infected by. 
After that first night, they can no longer be cured, they can take on their hybrid form or the form of the creature, at will but they will be forced into their hybrid form under the light of a full moon or when under intense stress.
