There's a lot of diseases you can get from doing stuff.

Contact

There's many ways to contact diseases.
Airborne: Whenever you're within 5ft of a target who has an airborne disease for three rounds or with 15ft of one target for at least a minute, you contact the disease.

Pox Fever

Pox fever is by far the most common illness throughout the world, while the mortality rate is overall low, it can easily kill someone who doesn't treat it right.
Stage One: Whenever you come into contact with the Pox Fever, you must make an Endurance Saving Throw, DC 15, on a failed save, you contract the Pox Fever. After 24 hours pass, your Endurance is reduced by -1 and you begin to feel the effects of the Pox. After every 24 hours after that, you must make another Saving Throw, DC 20. If you fail again, your Endurance is reduced by -1 and you have -4 to all mental skill checks. If you fail this save three times in a row, you move into stage two. Everytime you succeed a saving throw, your endurance increases back to 1, if it increases to the maximum, you are no longer sick.
Stage two: Whenever you fail a subsequent Saving Throw, your endurance is reduced by -1d3 instead of -1, you have -6 on all mental skill checks and -3 on all physical skill checks. If you fail another three saving throws in a row, move into stage three.
Stage Three: You have sustained permanent damage, whatever score your Endurance is cannot be increased from this number. You continue to make Endurance saving throws until you die.
