Reforged An old world shattered and remade into something new

Several millennia ago, a Great Cosmic War raged across Realmspace, home to Toril and its sister planets. The chaotic battles between sinister Outsiders and the ill-prepared deities of Toril tore the planet asunder. The War shattered the Prime Material Plane, sending ripples of devastation throughout the Cosmos. Some Gods survived; many did not. The world of Toril splintered into millions of fragments, while her sister planets suffered worse. Countless perished and in the ensuing years, many more would die as aftershocks reverberated throughout the Material Plane. Of the surviving Gods, little is known for it would be sometime before they found their way back into the hearts and minds of mortals.
As the dust settled, an eerie silence greeted the Survivors. Those fortunate few found themselves adrift where Toril used to be, clinging to life. Hundreds of years would pass before the shattered fragments of Toril began to drift together, as the planet somehow began slowly rebuilding itself. The other planets also drifted towards the old planet's remains, fusing with those remnants. Oddly, the Prime would also absorb aspects of those Planes closest to it, including the parallel planes of the Feywild and Shadowfel, as well as the Elemental Planes. The old planet would Reforge itself into something... new.
From the ashes of its former glory, Reforged heralded a new dawn. Survivors found themselves in a strange, yet familiar new world. For nearly a thousand years, Reforged's inhabitants struggled as they attempted to evolve with their new home. Although much was lost during the Great War, they found themselves eking out a meager existence against a harsh and savage new land.
Rebuilding a lost world has been wrought with challenges, as the peoples of Reforged compete against each other for resources amidst amazing discoveries and frightening dangeres. Bands of survivors founded new civilizations while others became mere memories. Conflicts, big and small, have drawn today's political boundaries, creating ripe opportunities for adventure. What will you find in these new lands? What will you become?
