Mouth of Sinners
Eventually your body will crave to be released from your sin. The sooner you step over the edge the sooner your body will be free.
When trying to collect information about the Underdark you will always run into a fundamental problem: As the caves shift, it sometimes becomes impossible to tell if the information was just a fabrication, or lost reality.
The mouth of sinners is one such feature that is still considered a lost reality.
A few shifts ago it was in the control of a brotherhood of priests, dedicated to cleanse the underdark of sin. They never made public what they'd consider a sin.
According to various sources, the mouth is supposedly a sinkhole of unknown depth that has a small shelf about 10 meters below the main cave system.
Anyone this brotherhood deemed sinful was lowered onto the shelf and then left there in the dark. Eventually people would either go mad from the lack of stimulation or desperate for relief from hunger and thirst. The only way to escape was the lip into the hole.
