It is filled with deadly traps, cunning illusions, and ferocious guardians poised to destroy the unwary. Yet, those who brave its depths do so for more than mere peril—for within its cursed halls, great treasures, both arcane and precious, lie hidden. But riches are not the only force lurking in the dark…

Legends whisper of an entity of unfathomable power that still wards his final haunt—the dreaded demilich Acererak. It is said that he wields magic beyond mortal reckoning, his very essence woven into the fabric of the tomb itself. Few who seek his resting place return, for the twisting corridors and death-laden chambers of the crypt are filled with poisonous mists, soul-draining wards, and traps devised by a mind untouched by mercy.

Even if a party somehow navigates the perils of the tomb, the demilich has concealed his true lair so completely that none are likely to find it. Only a band of seasoned warriors, cunning spellcasters, and the most resourceful of rogues stand even a sliver of a chance. Those who venture forth must prepare for failure, for in the Tomb of Horrors, the price of arrogance is eternal oblivion.


The Truth Behind the Legend

Ages past, a human wizard-cleric of surpassing evil sought to escape the shackles of mortality. He performed the most vile of rites, binding his soul into undeath and becoming the lich Acererak the Devourer. For many years, he ruled from his bleak fortress, surrounded by undead servitors and ghastly horrors in the very halls that now form his tomb.

Yet even the power of a lich is finite. When Acererak felt his essence waning, he set into motion his grandest scheme. For eight decades, his minions toiled, constructing the Tomb of Horrors, a labyrinth of exquisite and terrible craftsmanship. When their work was done, Acererak erased all trace of their existence, annihilating his servants so that none could betray his secrets. Then, he sealed himself away, allowing his body to fade, his soul untethered, free to roam unknowable planes beyond mortal comprehension.

Should foolish adventurers press deep enough into his lair, they will unwittingly call Acererak’s spirit back to the Material Plane. He will awaken, aware of the intrusion—and the punishment for disturbing his eternal rest shall be swift, merciless, and inescapable.


Acererak: The Devourer of Souls

Once a mortal, now something far worse, Acererak is no mere undead tyrant. His origins are steeped in darkness—born of a human sorceress and the balor Tarnhem, he was abandoned to die, only to be taken in by Vecna, the Whispered One. Under Vecna’s tutelage, Acererak embraced undeath as his ultimate destiny, rising to become a lich of legendary cruelty.

His enemies were many, from the holy paladin Pentivel to the righteous forces of Pelor, yet none could stop his rise. As he moved from mortal wizard to lich, then demilich, his ambitions grew ever more sinister. His followers, the Covenenticle of Acererak, sought to continue his dark legacy, but even they could not fathom the full extent of his plan.

In the wake of his departure to the planes beyond, his former acolytes splintered. Some became the necromancers of Skull City, who later swore allegiance to Vecna as the Votaries of the Black Spire. Others sought a means to restore Acererak to full strength, such as the githyanki necromancer Kastya Zurith-Movya, who believes the ancient demilich could aid his people in their endless war.


The Tomb of Horrors: Acererak’s Cursed Domain

For centuries, the Tomb of Horrors has stood as a graveyard of would-be heroes, its corridors littered with the bones of the arrogant and unprepared. It is a place of suffocating darkness, shifting illusions, and hidden death. The few who return from its depths do so broken in body and mind, whispering of horrors that defy reason.

As a demilich, Acererak no longer lingers in his tomb, but his influence remains. He now resides in the shattered Demiplane of Moil, a realm of eternal darkness and frozen ruins, where he furthers his grand design—a mystery that may yet shake the foundations of the multiverse.

Even still, his presence is felt across the planes. A shrine to Acererak, marked by a grinning humanoid skull, stands deep within the chaotic abyss of Pandemonium, where whispers of his return grow ever louder.

Acererak is more than a legend—he is a force of malevolence that cannot be destroyed. His schemes span across ages, and his tomb remains a deathtrap designed to ensnare the foolish, the reckless, and the overconfident.
