With the death of the previous local Great Ork, every Ork who wishes to claim the title engage in a bloody massacre of the others, until there is only one left standing.

Responsibilities

A Great Ork is the leader of the local group of Orks. The size of that group can range from a squad to a colony, to even the whole of the Orkyn State. They are responsible for all decisions made for the colony. Depending on the size of the group, they can appoint lieutenants to carry out specific decision making responsibilities.
