Targeting Body Parts of Big Monsters

When fighting certain big creatures, it is possible to target specific body parts in an attempt to injure them. Firstly, choose the part you wish to target; the DM has the final say on what you can target, but limbs, wings, tails, eyes, etc. are usually targetable.

The monster must be large or a bigger size category. The attack must use an attack roll to target a specific body part and you must declare you are doing so before the roll; the attack roll takes a penalty. This penalty can be -3 for easy, -5 for moderate, and -10 for hard; a limb would be easier to hit than an eye, per se. If the attack roll successfully hits the body part, it and the monster take the damage. If the attack roll is less than the AC, but would still hit the monster's AC without the penalty, the attack grazes the monster and it takes half damage.

A limb becomes injured when it cumulatively takes damage equal to 1/4 of the monster's health. The consequences of an injured limb are up to DM interpretation, though typically it means it can't use certain attacks or senses. Any magical healing heals the injury.
