Homebrew: Flanking

When making a melee attack, if a friendly creature is immediately adjacent to the target on the opposite side, the attacker gains a +2 bonus on their d20 check to hit.

Especially perceptive creatures cannot be flanked. If a creature has multiple heads, or it's passive perception is 18 or higher, it cannot be flanked.
