Effect

The target is imbued with magical energy and changes to a entirely different form. A willing creature morphs into a creature of the caster's choice. The target loses all physical traits, but retains mental skills, attributes and abilities. If the new form has spell-like abilities, they may not be used. Upon reaching zero hit points, the target takes damage equal to the hit points lost as the polymorphed form.
