Death and Resurrection

Death

When a character dies and is resurrected, part of their essence is forever lost. Their irises turn pale gray, almost white; also, the character has to roll on the indefinite madness table. These effect can only be removed using a Wish spell.
D100 Flaw
1-5 “Being drunk keeps me sane.”
6-10 “I keep whatever I find.”
11-15 “I try to become more like someone else I know—adopting his or her style of dress, mannerisms, and name.”
16-20 “I must bend the truth, exaggerate, or outright lie to be interesting to other people.”
21-25 “Achieving my goal is the only thing of interest to me, and I’ll ignore everything else to pursue it.”
26-30 “I find it hard to care about anything that goes on around me.”
31-35 “I don’t like the way people judge me all the time.”
36-40 “I am the smartest, wisest, strongest, fastest, and most beautiful person I know.”
41-45 “I am convinced that powerful enemies are hunting me, and their agents are everywhere I go. I am sure they’re watching me all the time.”
46-50 “There’s only one person I can trust. And only I can see this Special friend.”
51-55 “I can’t take anything seriously. The more serious the situation, the funnier I find it.”
56-60 “I’ve discovered that I really like killing people.”
61-65 “I'm convinced that I do not actually exist in this world and nothing I do has any real consequences.”
66-70 “I hunt and kill all members of a particular race or group of people I believe have wronged me.”
71-75 “I think every other creature is more powerful than me, and I treat them with either respect or fear.”
76-80 “Torturing others helps my pain go away.”
80-85 “I only have nightmares, no night will ever be quiet again.”
86-90 “I fear the darkness and I always carry a light with me. Being unable to see makes me panic.”
91-95 “I can't trust myself, or anyone for that matter, to make a decision. Only a coin can help me shape my fate.”
96-100 “Life is pointless and living its pleasures to the fullest is the only thing that matters.”

Resurrection

If a character is dead, and a return from death is attempted by a spell or spell effect with longer than a one action casting time, a Resurrection Challenge is initiated. Up to 3 members of the adventuring party can offer to contribute to the ritual via skill checks.

Resurrection Check

After all contributions are completed, the DM then rolls a single, final Resurrection success check with no modifier. The base DC for the final resurrection check is 10, increasing by 1 for each previous successful resurrection the character has undergone (signifying the slow erosion of the soul’s connection to this world). For each successful contribution skill check, this DC is decreased by 3, whereas each failed contribution skill check increases the DC by 1. Upon a successful resurrection check, the character's soul (should it be willing) will be returned to the body, and the ritual succeeded. On a failed check, the soul does not return and the character is lost. Only the strongest of magical incantations can bypass this resurrection challenge, in the form of the true resurrection or wish spells. These spells can also restore a character to life who was lost due to a failed resurrection ritual, should you allow it.

Quick Resurrections

If a spell with a casting time of 1 action is used to attempt to restore life (via the Revivify spell or similar effects), no contribution skill checks are allowed. The character casting the spell makes a Rapid Resurrection check, rolling a d20 and adding their spellcasting ability modifier. The DC is 10, increasing by 1 for each previous successful resurrection the character has undergone. On a failure, the character’s soul is not lost, but the resurrection fails and increases any future Resurrection checks’ DC by 1. No further attempts can be made to restore this character to life until a resurrection spell with a casting time higher than 1 action is attempted.
