Thiramin
History
Thiramin (Elvish for Soulmate) refers to two people, who, through an irreversible magical ritual, have intertwined their souls. Contrary to what the name might suggest, this bond isn't exclusive to lovers. In fact, the first mentions of this bond can be traced back to what is commonly referred to as The Age of Chaos, when two brothers are said to have forged a magical bond to help them defeat The Dark One.
This bond comes with many advantages, allowing Thiramin to communicate telepathically even over great distances. When apart, they can roughly tell in which direction their partner is and how far away they are. They are also able to tell the general emotional state of their partner and will feel their partner's pain to a certain extent. When close together, the Thiramin benefit from heightened senses. The longer ago the bond was created, the stronger the effect.
When apart, however, their senses will dull. There are records of Thiramin, who have become mad when separated from their partner for an extended period of time. When one of the Thiramin dies, the other can feel it. This usually results in the remaining partner following shortly after - or going mad. This is assumed to be caused by the absence of a part of one's own soul.
When a Thiramin dies, they cannot be resurrected by any means short of a Wish spell, since they do not possess their entire soul anymore. There are no written records of a Thiramin being resurrected with a Wish spell, so it is unknown whether there are side effects.
Execution
TW: Death, suicide
How the ritual is conducted in detail will always depend on the people forging this bond and what they mean to each other. However, there are two elements that are part of every ritual. First, both partners will ingest crushed Dreamroot, an extremely rare and expensive herb, which upon ingestion has the same effect as the Astral Projection spell. While in the Astral Plane they have to face a challenge, although non of the people who have successfully performed this ritual can remember what transpired in great detail. After having completed the challenge, the ritual is finalized by exchanging blood and saying the Thiramin Vows. While these Vows are the ones commonly used, the words themselves are not as important as the meaning behind them.
Before deciding to perform this ritual, it should be noted that there have been cases where one or both people attempting this ritual have died. If only one died, the survivor always went insane and committed suicide. The bond created by this ritual is irreversible, even with a Wish spell.

Never shall you wander in the dark again
For I will be the light that guides your way Sorrow shall never cloud your day again
For I will be the sun that brightens up your day And your hands shall never be cold again
For I will be the one holding them Your enemies shall be my enemies
As my enemies shall be your enemies
But they will never defeat us
For together we are stronger No hurdle shall ever be too high
For we will overcome it together And you will be mine as I will be yours
With this I vow my life to be yours
In life and in death - and for all eternity
