Eaora

The story of Eaora takes place 5,000 years after an global warming-induced apocalypse called the Resurgence. The civilizations of the world Before are no more, but with destruction comes new life, as the Resurgence reawakened the spirits of Eaora (dormant for ages within the earth, forests, and seas) alongside the birth of the Maje: humans who can wield the power of Vita that permeates all living and nonliving things in the universe. Humanity now lives within one of the Three Realms; continent-spanning nations that are descended from the remnants of the different countries, cultures, and peoples that existed Before the Resurgence. In the narrative present, the Three Realms are threatened by selfish humans who wish to undo the years of progress made by the Realms and malevolent spirits that wish to consume humanity in nature's reclamation of Eaora.
Eaora is divided into three major nations called Realms - Azuros, Vernal, and Orias. In the series, the word Realm is a unique term for a specific type of nation. Here, a Realm is defined as a continent-spanning country with a well-developed economy, a democratic government directly ruled by its common citizens, and (mostly) united diverse cultures.
Realms are essentially culminations of the remnants of different cultures and countries that used to exist on one of the three continents; they fell apart due to the chaos resulting from the global warming-induced apocalypse. Eventually humanity recovered a bit and Azuros was the first Realm created after its founders united the divided clans (based on the remnants of Eaora's ancient civilizations) of proto-Azuros (the founder's point and goal being that they were stronger together than apart). Their Realm forged a new cultural identity based upon all their various clans/cultures and served as a model for the cultures of Vernal and Orias (the argument being that by uniting as a Realm they could focus more on developing their societies than just simply surviving). But as can be expected, keeping order on a continent-spanning Realm is difficult, which is why Orias is the most divided Realm since it is the biggest and actually made up of three separate countries/cultures (they are all united at least by their common ancestry and some shared cultural traits). Vernal and Azuros are on smaller continents and thus a bit more easier to govern, but conflict still exists.

Geography

Varied, depends on which Realm and what part you're in. There are also many seas, a narrow ocean called the Traverse connecting the Three Realms, and the Yemoja Ocean - Eaora's largest ocean. In Azuros: coastal scrubland, rainforest, sandy desert, volcanic peaks. Vernal: northern island is largely jungle and tropical bush, southern island is largely a rocky/sandy desert Orias: the most varied and includes coastal pine forests, highland plains, sandy deserts, frosted tundra, tropical jungle, rainforest, and swamps.

This looks really cool so far, though I'd like to learn more about what makes a realm a realm and why.

I'm glad you like it and also thanks also for the follow btw! Sorry for the long paragraph of text but to answer your question, Realms are essentially culminations of the remnants of different cultures and countries that used to exist on one of the three continents; they fell apart due to the chaos resulting from the global warming-induced apocalypse. Eventually humanity recovered a bit and Azuros was the first Realm created after its founders united the divided clans of ancient Azuros (their point being that they were stronger together than apart). Vernal and Arzuros are on smaller continents and thus a bit more easier to govern, but conflict still exists. I hope that answered your question somewhat? I'm still working on this and I'm not so good at explaining my own things but let me know if you still have questions!
