The afflicted must decide whether they are targeting wounds, a poison, a disease, etcetera. The target must make a successful save versus spell at a penalty of 5 or suffer whatever the effects that the afflicted decides on.
Any wounds the target has will worsen, dealing an additional 1d4 damage for ever 5 hit points below maximum.
Any poisoning that the target has acquired has doubled in duration and effects.
The effects of a particular disease are doubled in strength.
