A small vial and a folded note sit inside a small box of bloodwood. Blood runs cover the surface of the box, and you hear a voice in your head saying to leave the box and another that says to open it.
Inside the box is a small vial of red fluid. It has no scent, and it flows like water. If a creature consumes the liquid inside the vial, they must make a Constitution saving throw DC 20. On a failure, the full effect of the liquid becomes released and takes effect immediately. The creature's base strength becomes 20, and their constitution becomes 18. They grow dog-like ears, and sharp claws that deal 1d8 damage, fur and a wolf's tail now grows on them. While the creature is under the effect of the vial they will attack any medium or larger creature they can see within 60 feet of them. This effect can be removed by the likes of remove cures, or greater restoration.
On a successful save the creatures back becomes covered in claw-like scars, their arms covered in black tattoos that depict bears and wolves hunting animals. The creature begins to grow dog ears and a tail, and most of its body gets covered by fur. One of the creature's eyes turns blood red, and the creature gains a +2 to Str and Con up to a maximum of 24. All melee attacks made by the creature get +1 and deal an additional 1d6 of poison damage. The creature gains proficiency in stealth and perception. Whenever the creature takes a long rest they must succeed a DC 10 Wis save, or else they go into a rampage for 10 minutes whenever they awaken. This effect can only be cured by going to the barren jungles of the Dog Folk and seeking a shaman who can cure it.
