The Act of Creation

  Fleeing from Bel'Sheggorath, the being known as Aeloah creates Candle'Bre as a pocket dimension to serve as a hiding place.   The creation of the plane involved stealing bits from the Prime Material Plane, Whilderhond (a Fey plane and home to the Spring Court), the Feywild (another Fey plane and home to the Summer Court), the Shadow Lands (a third fey plane and home to the Autumn Court) and the Shadowfell (last of the fey planes and home to the Winter Court).   In creating Candle'Bre in this way, Aeloah's pocket universe was tethered to all five via strands of magic, which both helped to nourish the tiny universe but also made it inherently unstable. The pocket universe was initially populated by creatures born of Aeloah's imagination and expanded from there.
