Pariah

Pariah vanguards are warriors who adopt radical self-sacrifice. Often martyrs or penitents, the strength of these defenders only grows the closer they come to death.

Painkiller

When you make this covenant at 3rd level, you swear to take others’ suffering as your own. You can use your reaction to Endure targeting an ally without expending a Focus die. If you do, you take damage of the same type as the triggering attack equal to the amount reduced. This damage is dealt as true damage to you instead.

Additionally, when an ally within the radius of your Timely Aid feature would be incapacitated, paralyzed, poisoned, or stunned, you can use your reaction to suffer the condition in their place.

Wellspring of Vitality

Also at 3rd level, Your life-force is bolstered by your willingness to sacrifice it. Your hit point maximum increases by 3, and by 1 whenever you gain a level in this class. You regain all your expended hit dice, rather than half, when you complete a long rest.

Aura of Suffering

Starting at 6th level, you can channel your pain into power. As a bonus action on your turn, you may activate or dismiss your aura of suffering. When you activate this feature and at the start of each of your turns while it is active, you and each creature of your choice within the radius of your Timely Aid feature takes psychic damage equal to one roll of your Focus die. While you are Bloodied (at half your maximum hit points or fewer) the damage dealt by your aura of suffering to creatures of your choice(but not you) increases by one additional damage die.

Grounding Focus

Also at 6th level, on your turn you can expend a focus die to immediately end one effect causing you to be charmed, frightened, incapacitated, paralyzed, poisoned, or stunned (no action required).

Empowering Martyrdom

No pain, no gain. Starting at 11th level, allies that start their turn within your aura of suffering gain temporary hit points equal to one roll of your Focus die plus your Constitution modifier.
Additionally, the damage of your aura of suffering increases by your Constitution modifier.

Cutting Agony

Starting at 17th level, psychic damage you deal ignores resistance. Additionally, whenever you take damage as part of your Painkiller feature, you can deal psychic damage to the attacker equal to the amount reduced as part of the same reaction.
