The Belt of Keyser Soze is a black nearly non-descript belt. Most of what is known about the belt feels to be a misdirection about how it is actually intended to be used.
If the wearer takes the shape of an animal and is reduced to 0 hit points, the wearer can teleport 150 feet from their location and revert to their human shape.
