Grave Worms
This spell is necromantic in nature and forces damage to the victim through infecting their body with worms from the grave.
Effect
When cast, the victim of the spell is infected with a number of grave worms causing damage when they burrow through the victim's tissue. These worms are driven into a frenzy due to being placed in a living body. These try to burrow out of the victim and if they are in the brain will cause mental trauma on top of any other damage.
It causes the victim 20 4d10 Necrotic damage.
Side/Secondary Effects
With the victim it leads to intense internal & external damage that affects the victim by damaging organs, many of them vital. The damage causes enough damage to permanently affect the victim even when cured or the spell wears off. Only other magic can repair the damage.
The victim must make a DC 20 Constitution saving throw after the damage roll is made. On a fail, 2 1d3 for a special necrotic point level, and further d6 1d6 . These necrotic points is took off the victim's Stat, running from 1 being Strength to 6 being Charisma. In other words the Stat is reduced permanently by 1, 2. or 3. ASI, wish spells etc can be used to restore it back to normal or improve them.
On a successful save this doesn't work.
Manifestation
The spell itself manifests by the crawling of worms throughout the body, eventually working their way rather quickly out of the body's orifices or organs. Eyes and skin is damaged in the process of the worms exiting. Even appearing out of some orifices can cause damage from the effect of these creatures going into overdrive to escape a still living warm body.
The whole time the victim can feel the worms moving and can cause extreme irritation. Many victims feel the need to scratch the area were worms are just below the skin, and anyone viewing them movement of them are revolted by seeing this occurring.

The spell's initial power is generally drawn from the grave of the recent dead. The dark magic that flows through the caster amplifies the effects and increased by the spite of the caster.
Discovery
Originally discovered by a lich called Shiva-Nergal in the City of the Dead based in the Ural Mountains, it has took many centuries to perfect the spell. Drawing on old sources from the time of the First Men during the Hyperborean Age, Shiva-Nergal realised just how potent it could be and now looking to improve further on it.
He has so far only passed it to some of his lackeys who shows a knack for targeting and torturing the living at a distance. Every time it has been used, they report back the to Shiva-Nergal the outcomes of each victim smitten with the spell.
Grave dirt with grave worms took from a grave of a once living sapient species that has laid dead for 3 to 6 months. Preferably one that has been executed or died a violent death, and one that has died by assassination is sought out above all else.
The dirt & worms are partially thrown and partially blown by the caster in the direction of the victim. The caster also says the command to Go Forth, Infect and Burrow Out. It can be hard for low level casters to learn as the spell needs the command and the blown portion to effectively be done at the same time for full effect.
