While attuned to the red orb, the player (and only the attuned player) can see a basketball sized floating orb-like creature with one eye.
The Ever Watching Eye floats near it's attuned owner always watching, waiting, expecting. Close inspection shows that the orb seems to be made out of organic material, as in organ-ic... When held you can feel the beating of an immense heart and a wave of sorrow overwhelms you as you hear the crying of a hundred souls.
All-Seeing-Eye: Grants the owner advantage on insight checks as the voice of the EWE informs them of deceit.
Blessing of the Heart: While attuned the owner may use an action to receives half their level of healing rounded down every round every turn for 1 minute.
Desperate Bargain: As an action the owner may bargain any number of their maximum hit points (returned after a long rest) for immediate healing of 2/3 the amount.
