Werewolf
The curse of the moon often called Lycanthropy. This affliction is contagious and afflicts those in small villages and groups. Often hunting alone isolated from society.
Transmission & Vectors
The Transmission is rare, the bite of someone already infected during the night of a full moon.
Causes
A human beaten and betrayed by bandits & society is left in the middle of the woods to die is let begging for his life. A wolf emerges but instead of eating him offers the human new life, so long as he bows his knee to the animal kingdom and converts other humans into beings like him. This newly born wolf-hybrid betrayed the wolf that gave him this blessing, refused to turn other humans into what he had become. Angering the Wolf, he was cursed to be a new creature, enslaved to the full moon. The moon caused his animal instincts to run wild, driving him to convert humans without a conscious thought.
Symptoms
Blackouts during the nights leading up to and on the full moon. Feeling weak on the night of the new moon. Strong cravings of meat are common though not necessarily guaranteed.
Treatment
If it is still progressing, a lesser restoration spell or equivalent could cure it, while if the werewolf form has taken, then it qould require a greater restoration (from level 5) on the night of a new moon. There are rumors of a herbal remedy that could cure it as well though you haven't found any text that supports this yet.
Affected Groups
Generally, there are more male werewolves, as if a pack attacks a village, the men would generally try and protect the women. Though there are women werewolves, and they are arguably more ferocious and wild than the men. The most dangerous though is a child that has been burdened with the curse, as they often can't control themselves even on knights near the full moon.
Hosts & Carriers
Any child naturally born of two werewolves is a carrier of the disease until they reach puberty.
Cultural Reception
Generally, the cultural reaction is one of two, acceptance from other wolves or fighting for territory & hunting ground. Or disgust from general humans.
