The Planar Manual of Kaelat

Like many multiverses in the various incarnations of D&D, Kaelat follows the basic Prime/Inner/Outer model of planes. The Prime Material plane is directly adjacent to its two sister planes, the Shadow Plane and the Feywild. The three planes are laid on top of each other, each just out of reach of the others. Minor magic can transport you to one or the other, and creatures of the other planes commonly “bleed” into the Prime Material.
Surrounding these three planes is the Ethereal plane, the plane of spirits and dreams. The Ethereal plane surrounds and binds the three Inmost planes together. The three Inmost planes are, after all, merely “dream” versions of each other.
The Inner Planes-- the four Elemental planes, the two Energy planes, and the multiple Demi-, para-, and quasi-elemental planes-- are all contained here. They connect to the Inmost planes via the Ethereal plane.
The Soulstream runs through the Inner planes before reaching the junction of the Inner and Astral planes, the Nexus Prime. Here, at the Nexus Prime, the soul is separated from its life energy, and the life energy is sent either to the Positive Energy Plane or the Negative Energy plane, while the soul itself is sent to a corresponding outer plane.
The problem is that the Astral Plane no longer connects to the Inmost planes (the Prime Material, the Feywild, and the Shadowfell). The only way the outer planes connect to the Inmost planes is through key junctions of an outer plane and one of the inner planes. The Negative Energy plane is strongly connected to the evil planes, and the Positive Energy plane is strongly connected to the good planes. The Planes of Fire and Air are strongly connected to the chaotic planes, and the Planes of Earth and Water are strongly connected to the lawful planes. This is part of why Kaelat deities are directly tied to elements; their manifestations are only possible when combined with the corresponding inner plane. As such, Outsiders are commonly given elemental templates when summoned. Because the Astral Plane is no longer directly connected to the Prime Material, teleportation and other forms of Astral movement and/or communication are impossible. However, in the Inner Planes, all of the above are commonplace.
