Far Realm

A place so synonymous with madness that everything "known" about it might be an hallucination. What is relatively well-established is that this is the birthplace, or at least the spiritual origin, of most aberration creatures.
Fortunately, portals or transitions to and from the Far Realm are rare, except between it and the demiplanes collectively known as The Abyss.
