When not in use, it can go into hibernation mode so it can survive without food for a long time. However, if it goes without feeding for several decades, it can die.

The hirudo are blood-drinking leech-like creatures that look like sharpened bone & cartilage fist coverings. They wrap around the wielder's knuckles and wrists, and have claws extending out like a punching dagger. They count as a monk weapon.
The hirudo form a symbiotic link with their wielder. Once attuned, each time the hirudo deal damage, the wielder gains half of the damage dealt as healing, and their max hp goes up by the same amount. After the wielder has received increased max hp from this weapon five times, their hp maximum stops increasing, however they can continue to heal damage taken using this weapon. The max hp increase and cap resets at a short or long rest.
