Stone of Kelemvor

The stone kelemvor is a magical stone created by a long ago group of wizards within the wizards cove. This specific group of wizards loved the goddess of death herself and devoted their entire lives to understanding death and its power, furthering themselves in necromacy with hundreds of failed attempts. Eventually learning to channel said energy through a magical stone that was a gift from the goddess herself through a lifetime of devotion.
With the power of the magical divine object they were each able to face a challenge or a test from the goddess, determining whether they were worthy of coming back to the material plane or would be ferried to the other side of the veil.

Mechanics & Inner Workings

The stone of Kelemvor is an ancient and powerful magic item that posesses powerful resurrection magic. 
On failing 3 death saving throws, the holder is transported to the astral plane where they are faced with a physical manifestation of Kelemvor, the deity of Death and Judgement. The holder fights the aspect of death with their max HP and spell slots available, as if they had just completed a long rest. If the holder of the stone defeats the aspect of death, they are resurrected at half of their max HP, with the same number of spell slots available as they had when they died.
