Ulthrosh Kadanis

Ulthrosh was one of the Archmages of Acopolis Scientia back in the day, he researched immortality and devised potions of Longevity. But the day Yog-Sototh arrived most of the city went mad and the city tore itself apart. Critically wounded Ulthrosh hastily devised a way to transfer his soul to a jar and keep on living. After his transformation into a lich was complete he conquered the city and slayed its mad inhabitants only to be raised as undead under his command.
