Endure Sewage Maintenance Worker

The City of Endure, capital of Dhalmain, is the proud creator of the frist properly planned and interconected sewage & plumbing system in the archipelago.
It was designed with the expansion of the city in mind and made to last for as long as possible, so it provides a long network of tunnels, big enough to travel on foot, that requires maintenance to keep safe and functioning.   Of course, such a network out of the view of the public and with lots of access in and out through all the cities couldn't have been a more convenient place for all kinds of pests to lurk in, including these of the same species of their creators.
The Underfolk, a loose organization of criminals and other scoundrels formed as their members figured that it was best to not try to kill each other too much while occupying these tunnels and that they needed to keep the authorities, if not oblivious of their existence, at least uninterested in their removal.   And for that, they need the sewage maintenance workers to cooperate.  

Weird Luxury

The Endurite sewage workers are paid quite well by the state to do their undesirable job, but some of them seem to live with an amount of luxury that doesn't match their salary and an amount of stress that doesn't match their official tasks.   These, the lucky ones, usually are tasked with the "maintenance" of some specific areas of the network that are in "need" of constant repairs, for which they usually request important amounts of resources and founding insisting that the section may crumble at any point if not properly repaired, and so nobody should go close to them save for the people doing the repair work.  

The Tongue-eating disease

Some poor maintenance folk get afflicted with a terrible disease specific to this sewage network, known as the tongue-eating disease.
These afflicted say to be in so much pain that they are unable to leave the pipes for a day or sometimes more, and when they are back they are missing chunks (or the entirety) of their tongues, which they explain to have rotten down and fallen in the course of the day.   Sometimes, the tongue-eating disease affects only one or more fingers instead.   In any case, the afflicted become sombre and terrified afterwards, and usually refuse to talk about their experience.

Tongue eating disease?! Well, I'm certainly never going into these sewers. AND I will properly value these workers -- even if some of them are living a strange life of luxury.

A creative way to show workers adjacent to more seedy folk — much to gain. Maybe more to lose. Am I correct to assume the tongue-eating disease may have to do with the underfolk not happy with a worker who talks too much?
