Cult of Gruumsh
Superiority is determined by strength and true leadership can be determined by body count. Gruumsh teaches his followers that violence is always a solution for solving an issue. It might not always be the best solution, but it is always there to be considered. And even other approaches is better brutal than gentle.
Those who follow Gruumsh sacrifice to him on a regular basis, normally by killing an animal and painting themselves or a crude altar with its blood. Often, this is done in order to have fortune with some fight or some form of hard physical trial, but especially for orcs, this is done as a general form of worship.

Structure
There is no structure to speak of beyond each clan. Orcs tend to have at least one, but up to a hundred, shamans dedicated to Gruumsh, but they hold no authority outside the tribe. Outside orcish societies, worship of Gruumsh is extremely rare, and temples are run by individual zealots and possibly a pair of acolytes.
Public Agenda
Most of those who don't follow Gruumsh, fear him. The brutal violence he encourages is detrimental to any civilised society.
