Anatomy
A massive floating eye with 2 or 4 celestial wings on it's back to keep the eye moving.
Growth Rate & Stages
The eye of the beholder seeks a place to rest, when it rests long enough(which is often 1 to 2 years) it can regenerate it's beholder body.
Ecology and Habitats
They are very rare but could be practically everywhere, nothing keeps the eye from migrating, except extreme heat and cold places.

Deals 1d4+6 psychic damage, and reduces the targets movement to 0 until the eye's next turn.
Banisment
INT 16 banishes the target to a pitchblack pocket dimension. the target stays there until the eye dies or wills it to return. the eye can only have 1 creature in his pocket dimension at once.
Psychic wave
deals 4d4+6 psychic damage. and 1 stress damage.
Wave of madness
[AOE](?)everyone within 30 ft of the eye turns made for 5 turns, the DM can move these creatures randomly around the map until they regain sanity. also deals 2 stress damage.
Wing storm
[AOE] (?) a 60ft + or x that deals 2d12+1 damage and pushes targets back for 30ft, throwing them prone.
