This table is the last chance to cheat death, but not without constantly being reminded of your near death experience. The only way to undo the traumatic experience is with a Wish spell.

2	Lose an arm	-1d4 STR and -1d4 DEX. Can no longer wield 2-handed or duel wield.
3	Robbed while unconscious	Lose ALL items on person and in the Bag of Holding
4	Major concussion	Instantly lose a level.
5	Scattered nerve atrophy	Lose proficiency in martial weapon, medium armor, heavy armor and shields.
6	Long term memory loss	Lose proficiency in 4 skills.
7	Develop Neurosis	-1d6 CHA
8	Develop debilitating arthritis	Lose your 1st level feat.
9	Develop an autoimmune disorder	-1d6 CON
10	Major leg injury	Movement speed reduced to 20 feet.
11	Acute nerve damage	-1d6 STR
12	Develop peripheral neuropathy	-1d6 DEX.
13	Slight concussion causes dementia	-1d6 INT
14	Major spleen trauma	Lose 30% of total health. (rounded up)
15	Acquire vertigo	Can no longer wield 2-handed, shields or duel wield.
16	Short term memory loss	Lose your last learned feat
17	Concussion causes cloudiness	-1d6 WIS
18	Stroke	Unlearn all languages except Common
19	Minor concussion causing sporadic weakness	Lose 1 proficiency of the player's choice
20	Small scar	None
