Uncommon Transmutation
When you drink this potion, roll a Constitution Saving Throw. The DC is 10 plus the number of times you have taken this potion. On a success, you can choose one of the following benefits:
- +2 to hit and damage with weapon attacks for one minute
- +2 to Spell attacks and Spell save DCs for one minute
- +2 on skill checks for the next 10 minutes
- +2 on initiative rolls and Armor Class for one minute
- You gain 3d8 temporary hit points and +10ft of movement speed for one hour.
On a failure, you take 1d8 poison damage for each time you have used this potion and are poisoned for 1d6 minutes. If you fail by more than 5 points, you are instead poisoned for 1d6 hours.
The effective number of doses taken can be reduced by 1 with the help of a lesser restoration spell, by 3 with a greater restoration spell, or fully reset with a wish spell.
