Rare Transmutation

When you drink this potion, roll a Constitution Saving Throw. The DC is 10 plus the number of times you have taken this potion. On a success, you can choose one of the following benefits:

• +3 to hit and damage with weapon attacks for one minute
• +3 to Spell attacks and Spell save DCs for one minute
• +3 on skill checks for the next 10 minutes
• +3 on initiative rolls and Armor Class for one minute
• 3d10 temporary hit points and +15ft of movement speed for one hour.

On a failure, you take 1d10 poison damage for each time you have used this potion and are poisoned for 1d6 minutes. If you fail by more than 5 points, you are instead poisoned for 1d6 hours.   The effective number of doses taken can be reduced by 1 with the help of a lesser restoration spell, by 3 with a greater restoration spell, or fully reset with a wish spell.
